Stages of Diabetic Angiopathy Progressing to Gangrene
Diabetic gangrene does not develop overnight. It represents the final stage of a prolonged process of vascular deterioration that typically progresses through the following stages:

Chronically elevated blood glucose levels damage the inner lining of blood vessels (the endothelium), leading to ongoing inflammation and vascular dysfunction.
2. Microangiopathy
Excess glucose and its metabolic by-products contribute to narrowing and damage of the small blood vessels supplying the lower limbs, kidneys, and eyes.
3. Ischemia and Hypoxia
Blood flow becomes significantly reduced. The feet may remain persistently cold, pain may occur even at rest, and wounds heal slowly or fail to heal altogether.
4. Tissue at Risk (Pre-Gangrene Stage)
Dark spots may appear on the heels, while the toes can develop a bluish discoloration, indicating the onset of severe tissue compromise.
5. Established Gangrene (Necrosis)
Tissues become blackened and die. At this stage, there is a substantial risk of serious infection and sepsis. Without urgent medical intervention, amputation may become necessary.
