Knee osteonecrosis, also known in the international medical community as aseptic necrosis or avascular necrosis (AVN), is a particularly serious and painful pathological condition. It occurs when, for various reasons, the microcirculation and blood supply to a specific area of bone are interrupted or dramatically reduced—most commonly in the medial femoral condyle or, less frequently, in the tibial plateau.

Prolonged deprivation of oxygen and essential nutrients inevitably leads to the gradual death of bone cells (bone tissue necrosis). If the condition is not detected and treated promptly, the subchondral bone loses its structural integrity, resulting in progressive collapse, accelerated secondary osteoarthritis, and ultimately complete joint destruction.

Clinical Presentation & Symptoms

Knee osteonecrosis develops through distinct stages, with symptoms becoming increasingly restrictive to the patient’s daily life:

• Acute & Localized Pain

A sudden onset of severe knee pain (typically on the inner side of the knee), which worsens significantly during weight-bearing activities such as standing and walking. In later stages, the pain may persist even during rest or sleep.

• Joint Swelling & Inflammation

Persistent swelling (joint effusion) may develop around the knee as a result of reactive synovitis and increased intraosseous pressure.

• Progressive Stiffness

A significant reduction in the joint’s range of motion, making everyday activities such as deep squatting or climbing stairs increasingly difficult.

• Mechanical Instability & Limping

Patients may experience a sensation that the knee is “giving way,” accompanied by difficulty maintaining a normal gait pattern and the development of a limp to offload the affected area.
